FEE: $300 INCLUDES This consult includes your entire health history where options and solutions are discussed. This phone session lasts 90 minutes. This consult is for those who have many or complicated health issues.

GETTING STARTED: There are several health history forms that will be e-mailed to you in an Adobe Acrobat™ pdf file. Fill out the forms and then fax or snail-mail the completed forms back to me. Never e-mail your completed health history forms. Once I have receive all your information we can have the consult. Depending on how quickly you return the forms, depends on how quickly we can have the consult. Please use the outline under 'Signs and Symptoms' when describing your symptoms to me.

A. I respect your privacy, since you may not want others in your household to know about the phone consult, send me an e-mail or phone me at 314-420-5099 to schedule the consult. You can do this before or immediately after ordering. We will schedule the time and I will call you at the time, date and number you desire. Unless of course, you would feel more comfortable calling me at the time and date you desire.

• Call to and ask any questions about the appointment before payment
• Once payment is made, we will schedule the appointment that fits both our schedule
• All personal consults take place over the phone or using SKYPE and lasts 90 minutes.
• I will send your the usual "doctor paperwork" through e-mail. This will be the usual paperwork you would receive if you were to walk into my office that includes medical history, family history, any medications or supplements you are taking, etc.
• Fax or mail your paperwork back to me. NEVER EVER e-mail your doctor paperwork.
• Naturally a physical exam is not possible.
• During this consult I will help you put your health picture together and point you in the direction of health, growth and healing or interpret any lab results or information that you have for me to interpret.
• If you pay by e-check, it may take up 3-7 days for this e-check to clear and the personal consult will not take place until after the e-check has cleared.
